Job Details
This role is focused on psychiatric consults for patients in skilled nursing facilities and assisted living communities.
Responsibilities include:
Perform psychiatric evaluations and follow-up visits Manage medication treatment plans Collaborate with facility staff, physicians, nurses, social workers, and families Provide consultative behavioral health support to post-acute and long-term care residents Document visits and treatment plans in a timely manner Travel to facilities throughout New London County
Qualifications:
Licensed Psychiatric Nurse Practitioner in Connecticut PMHNP certification required Experience with geriatric, long-term care, SNF, assisted living, or post-acute populations preferred Comfortable working independently in the field Strong documentation and communication skills
